Did Rife use radio or audio frequencies for his work?

Every Rife machine used carrier frequency, anything from 28 MHZ to 2.3MHz (in different time frames.

While the Rife #4 machine could emit two radio frequencies at once, it did not use a carrier wave. Here are historical images of the frequency lists:

The #4 machine had to have been a pretty effective machine, because that is the one he used in the 1939 lab film, which demonstrated effects on pathogens.

There may be more than one way to achieve effects.
